This is a combination of two Kensington linear stages to form an XY stage. The lower linear is of the 8500 series, and the upper of the 6000 series of stages. The model number is 8500-6×6000-4(-). Specifications include:

The stage is in excellent condition. Both axes work very smoothly. It appears that the two cover plates for the belt drives are lacking, but these do not affect operation. Both stages have optical encoders installed.
